Job Summary

We are seeking a skilled Telephonic Triage Nurse to join our team. As a Telephonic Triage Nurse, you will be responsible for assessing callers' symptoms, prioritizing calls, and advising patients or families on basic treatment or sending a nurse to evaluate the patient's situation.

Key Responsibilities
  • Receive and respond to calls for assigned Hospice service area
  • Collect and analyze data to make informed decisions related to patient care
  • Prioritize patient care situations, providing information, education, support, and continuity of care
Requirements
  • Graduate of an accredited school of nursing
  • Current Registered Nurse (RN) licensure within a NLC Compact State
  • Three (3) years clinical experience
  • Minimum of two (2) years' experience in oncology, long-term care, or hospice or home health
  • Prior experience with telephone triage and Electronic Medical Records (EMR)
