Job Summary

Millennium Physician Group is seeking a skilled and compassionate Physician Triage Specialist to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and ensuring the smooth operation of our triage processes.

Key Responsibilities
  • Participate in the After-Hours On-Call Team rotation, providing timely and effective support to patients and healthcare providers.
  • Answer all incoming calls immediately, providing accurate and compassionate advice to patients and their families.
  • Provide Post-Acute Care Outreach, ensuring seamless transitions of care and optimal patient outcomes.
  • Assist Hospitalist's Patient Outreach, facilitating communication and coordination between healthcare providers and patients.
  • Facilitate Prescription Refills, ensuring timely and accurate medication management.
  • Assist RN Care Management Teams with patient assessments and/or monthly follow-up calls, when needed.
  • Monitor Patients enrolled in the Patient Monitoring Program, providing proactive and personalized care.
Requirements
  • Registered Nurse licensed in FL, with a valid license always kept current.
  • 5+ Years of clinical experience, with 1+ year(s) of case management experience preferred.
  • ER/ICU/Triage experience is preferred.
