The Triage RN Coach plays a vital role in our telephonic virtual care center, working collaboratively with healthcare professionals to provide exceptional care across the continuum. This position is an integral part of the Care Connections Team, ensuring safety, best practices, and high-quality standards are maintained.
Responsibilities- Assess patient needs through telephone triage, utilizing approved protocols to provide relevant health information.
- Support holistic, interdisciplinary patient-centered care, promoting effective management of acute and chronic illness.
- Assist patients in navigating the healthcare system, ensuring they receive the appropriate level of care.
- Answer a nurse triage line, responding to general health questions and counseling patients as needed.
- Collaborate with providers, receiving and relaying radiology and lab results, and encouraging patient engagement to reduce the risk of inappropriate care and hospitalization.
- 4-year Bachelor's Degree, required.
- 5 years of clinical experience in a hospital, home health, ambulatory, or community setting, with a focus on acute and/or chronic-care management, required.
- Current unrestricted RN license for the states in the NH footprint, required.
