Job Overview

We are seeking an experienced Social Worker to join our team at Judson, where you will play a vital role in providing exceptional care and support to our residents.

This is a full-time position working 40 hours per week, with a competitive salary of $60,000 per year.

Responsibilities
  • Casework and Counseling: Provide ongoing casework services, including interviews, crisis intervention, grief counseling, psychosocial assessments, information and referral, discharge planning, suicide risk assessments, and advocacy.
  • Social History and Documentation: Complete social histories and update resident information, ensuring accuracy and compliance with licensure requirements and professional social work standards.
  • Interdisciplinary Collaboration: Participate in interdisciplinary care planning, completing quarterly care plan documentation and progress notes in the medical record.
  • MDS Assessments and Tracking: Complete necessary MDS assessments and maintain tracking documentation.
  • Benefits Assistance: Assist residents and families with application and eligibility for Medicaid and other assistance programs.
  • PASSAR Screens and Levels of Care: Ensure timely completion of PASSAR screens, PAS IDs, and levels of care, placing them in the correct section of the resident's medical record.
  • Paperwork Preparation: Prepare papers for submission to Medical Advisory Committee.
  • Family Meetings and Clinical Audits: Conduct family meetings to prepare residents and families for level of care changes, and conduct clinical audits as deemed appropriate by the director.
  • Administrative Support: Attend and participate in weekly, monthly, and quarterly administrative meetings, standing or ad hoc committees pertinent to the position.
  • Record-Keeping and Teamwork: Maintain resident social work records and files, provide coverage for colleagues when needed, and work collaboratively with the social work team and interdisciplinary team to accomplish departmental and organizational goals.
Required Skills and Qualifications

To be successful in this role, you will need:

  • Ohio Licensed Social Worker (LSW) certification.
  • A Bachelor of Social Work (BSW) degree.
  • Three years of experience in a gerontological setting.
  • Excellent computer skills.
