Key Responsibilities:


- Conduct initial assessments and follow-up consultations in various settings including hospitals, rehabilitation centers, and outpatient facilities.
- Evaluate symptom distress and develop treatment strategies to alleviate suffering.
- Engage in advanced care planning discussions, providing recommendations and necessary documentation.
- Facilitate family meetings to discuss care options.
- Offer guidance on behavioral modifications and techniques to minimize health complications.
- Address inquiries from patients, families, and interdisciplinary team members regarding treatment alternatives.
- Ensure accurate documentation and billing for services rendered.

- Lead educational sessions for staff and community partners to enhance awareness of supportive care criteria and issues.


- Provide on-call services as required.
- Cultivate and maintain relationships with hospital and facility personnel.
- Collaborate with private practice providers to strengthen referral networks.
- Authorize prescriptions electronically and complete necessary documentation for patient care.

Qualifications:

- Master's degree in nursing with a focus on Adult, Geriatric, Family Medicine, Oncology, or related fields.
- Active Nurse Practitioner license in Maryland and registration for controlled substances.
- Board certification in Hospice and Palliative Care is required within two years of employment.
- DEA registration must be in good standing.
- Passion for educating future healthcare providers, including nurse practitioner students.
- Minimum of two years' experience caring for seriously ill adult patients.

- Strong understanding of Palliative Care principles and familiarity with hospice and home care services.


- Proficient in ethical and legal aspects of end-of-life care and controlled substance prescribing.
- Excellent communication, organizational, and interpersonal skills are essential.
- Basic computer proficiency, including experience with electronic medical records.
- Valid driver's license in good standing is required.
