Kindful Health LLC is seeking a highly skilled and compassionate Hospice Clinical Liaison to join our team in San Antonio. Key Responsibilities:
- Generate qualified referrals to achieve hospice admissions and meet target account goals.
- Maintain accurate records of referral sources, including physicians, hospitals, and senior living facilities.
- Assist care centers in obtaining necessary documentation for the referral to admission process.
- Evaluate patient referrals to determine appropriateness for admission to hospice.
- Provide consultation and counseling to referrers, patients, and families about Kindful's hospice care options.
- Communicate patient health, financial, and medical needs to the care team and coordinate resources to meet patient needs.
- Act as a liaison to patients and families during the transitional period between discharge and initiation of hospice services.
- Develop and maintain client relationships within a competitive marketplace.
- Establish and promote ongoing collaborative relationships with referral sources and institutional personnel.
- Prepare business plans and maintain target lists.
- Respond to customer complaints in a timely manner.
Requirements and Qualifications:
The ideal candidate will have a strong understanding of hospice care and excellent communication skills. A minimum of two years of hospice sales or clinical experience is required, along with a valid LVN or RN licensure. The successful candidate will be happy in the field, providing patient care, and embracing new opportunities to learn and grow.
