Hospice Care Nurse Specialist

As a key member of our interdisciplinary team, you will provide direct patient care services to hospice patients and supportive care to their families after normal business hours.
Responsibilities include:
* Evaluating patient/family needs and assisting in the preparation and execution of the plan of care for each patient
* Maintaining all assigned patient clinical records
* Participating in company quality assessment and performance improvement programs
* Managing an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance the quality of hospice care
