Per Diem Hospice Social Worker

We are seeking a compassionate and skilled Social Worker to provide comfort and support to patients and their families facing end-of-life care. As a family-owned and operated Hospice, we are dedicated to exceeding the expectations of our patients and their families by embracing excellence in all that we do.
Key Responsibilities:
- Implement standards of care for medical social work services
- Assess the psychosocial status of patients and families/caregivers
- Carry out social evaluations and provide counseling as needed
- Maintain clinical records and provide information and referral services
Requirements:
- Graduate of a bachelor's program in social work accredited by the Council on Social Work Education
- Minimum of one year's experience in health care, hospice experience preferred
- Good verbal and written communication skills
- Possess and maintain current CPR Certification
