About the Job

Job Summary: As an RN Case Manager, you will be responsible for conducting initial client assessments, developing care plans, and coordinating patient services. If you are passionate about delivering exceptional patient care and have strong communication skills, we encourage you to apply.

Main Responsibilities:
  • Develop and implement personalized care plans that meet individual needs
  • Collaborate with interdisciplinary teams to ensure coordinated care
  • Provide emotional support and counseling to patients and families
  • Manage assigned caseload and maintain accurate records
Requirements:
  • Licensed as a Registered Nurse in the state of employment
  • Experience in hospice care or similar settings preferred
  • Strong organizational and time management skills
  • Ability to work autonomously and as part of a team
Remuneration and Benefits:

The estimated salary for this position is $75,000 - $95,000 per year, depending on experience. Our benefits package includes health insurance, paid time off, and opportunities for professional growth and development.
