Care Coordinator

We are seeking a dedicated and compassionate Care Coordinator to join our team at North Homes Inc. in [Location]. This role will play a crucial part in ensuring seamless care for our clients by connecting them with tailored services based on their unique needs.

Key Responsibilities:
  • Assess client needs through intake and preliminary screening processes.
  • Link clients and/or families with relevant services and resources.
  • Maintain continuity of care among the client's treatment team.
  • Foster collaboration between internal and external providers to develop person-centered integrated treatment plans.
  • Identify and assist clients with Social Determinants of Health (SDOH) issues.
  • Collaborate closely with internal and external referral sources.
  • Participate in weekly intake rotation for new inquiries and referrals.
  • Support clients with updating intake paperwork and ROI annually.
  • Maintain accurate and timely care coordination summaries.
  • Manage scheduling, record disclosure, and tracking referrals for assigned providers.
  • Monitor Encounter Alert Service (EAS) and follow up with clients within 24 hours of discharge.
  • Collect and enter required data into the Electronic Client Record (ECR).

Requirements:
  • A high school diploma or equivalent is required.
  • The individual must be non-judgmental regarding lifestyle, race, and economic status.
  • They must pass a DHS background study and meet applicable requirements and regulations.
  • A valid Minnesota driver's license is necessary, as well as being at least 21 years old.

PREFERRED EXPERIENCE:
  • Knowledge in behavioral health programming is desired.
  • Two years of college in a healthcare or human service field, or three years of experience in a similar setting, preferably within a behavioral health clinic or social services.

  • Proficiency in using an Electronic Medical Record (EMR) system.
  • Compassion, empathy, caring, and understanding towards individuals and their families' circumstances.
  • Professional collaboration with internal and external providers and agencies.
  • Maintenance of confidentiality.

Physical Requirements:
  • Prolonged periods of sitting at a desk and working on a computer.
  • Lifting up to 15 pounds may be required occasionally.
