Key Responsibilities:
- Assist individuals in acquiring, maintaining, or enhancing skills through engagement in various daily activities, both at home and in the community, to foster greater independence.
- Implement personalized support strategies to help individuals achieve their unique goals.
- Maintain accurate and thorough daily progress documentation for each shift worked with individuals.
Qualifications:
- Bachelor's Degree or a minimum of 2 years of relevant community-based experience with adults with disabilities.
- Valid driver's license and current insurance.
- Willingness to transport individuals in personal vehicles throughout the community.
- Strong organizational, interpersonal, and communication skills.
