Job Summary

Theoria Medical is seeking a skilled Nurse Practitioner to join our team as a Primary Care Provider for Skilled Nursing Facilities. As a key member of our post-acute care team, you will be responsible for providing high-quality, patient-centered care to residents in skilled nursing facilities.

Responsibilities
  • Provide comprehensive primary care services to patients residing in SNFs, including rounding, prescribing medications, and treatments.
  • Manage the clinical care of the population at the assigned SNF, working closely with facility nursing staff to ensure timely and effective care.
  • Document patient care and encounters in Theoria's proprietary electronic health record (EHR) system, ChartEasyTM.
  • Be available for phone/secure messaging consultations with facility nursing staff to address acute/emergent issues.
  • Participate in leadership opportunities and contribute to the growth and development of our team.
Requirements
  • Current licensure as a Nurse Practitioner in the state of practice.
  • Experience working in a skilled nursing facility or post-acute care setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with facility staff and patients.
  • Ability to work independently and as part of a team, with a focus on providing high-quality patient care.
