We are seeking a skilled Nurse Practitioner to join our team at Theoria Medical, a comprehensive medical group and technology company dedicated to serving patients across the care continuum.
Job Summary:The successful candidate will be responsible for providing high-quality, patient-centered primary care services to patients residing in skilled nursing facilities (SNFs). This role requires a strong clinical background, excellent communication skills, and the ability to work independently in a fast-paced environment.
Key Responsibilities:- Rounding and managing the clinical care of patients at assigned SNFs
- Prescribing medications and treatments for residents residing at SNFs
- Providing timely documentation of all patient care/encounters
- Available for phone/secure messaging consultations with facility nursing staff
- Valid PA/NP license in the state in which they will practice
- Valid NP/PA degree from an accredited institution
- Active DEA license (Theoria will assist with application for new graduates)
- General Post-Acute/SNF knowledge or experience preferred
- wRVU productivity
- Malpractice insurance covered by employer
- Opportunities for training (i.e. wound care)
- Equipment not provided
- All practice expenses covered
