Job Title: Nurse Practitioner

We are seeking a skilled Nurse Practitioner to join our team at Theoria Medical, a comprehensive medical group and technology company dedicated to serving patients across the care continuum.

Job Summary:

The successful candidate will be responsible for providing high-quality, patient-centered primary care services to patients residing in skilled nursing facilities (SNFs). This role requires a strong clinical background, excellent communication skills, and the ability to work independently in a fast-paced environment.

Key Responsibilities:
  • Rounding and managing the clinical care of patients at assigned SNFs
  • Prescribing medications and treatments for residents residing at SNFs
  • Providing timely documentation of all patient care/encounters
  • Available for phone/secure messaging consultations with facility nursing staff
Requirements and Qualifications:
  • Valid PA/NP license in the state in which they will practice
  • Valid NP/PA degree from an accredited institution
  • Active DEA license (Theoria will assist with application for new graduates)
  • General Post-Acute/SNF knowledge or experience preferred
