Job Summary

We are seeking a highly skilled and compassionate Social Services Specialist to join our team at Marshwood Center in Lewiston, ME. As a key member of our interdisciplinary team, you will work closely with patients, residents, and their families to identify and address their social, emotional, and mental health needs.

Key Responsibilities:
  • Conduct Comprehensive Assessments: Complete thorough psychosocial assessments for each patient/resident to identify their strengths, needs, and goals.
  • Develop and Implement Care Plans: Collaborate with the interdisciplinary team to create and implement individualized care plans that address patients'/residents' social, emotional, and mental health needs.
  • Provide Education and Support: Educate patients, residents, and their families on available resources, services, and support systems to promote their overall well-being.
  • Facilitate Discharge Planning: Identify and address discharge goals and needs, ensuring a smooth transition for patients/residents to their next level of care.
  • Communicate Effectively: Maintain open and effective communication with patients, residents, families, and healthcare team members to ensure seamless care coordination.
Requirements:
  • Education: Bachelor's degree in social work or a related human services field.
  • Certifications/Licenses: Possess any required certifications or licenses to practice in long-term care, as per the State of employment.
