We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a key member of our interdisciplinary team, you will provide exceptional hospice care to patients and families, upholding our high standards of nursing practice.
Key Responsibilities:
- Maintain a specified case management load.
- Conduct initial and ongoing assessments of patients' physical and psychosocial status.
- Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG).
- Coordinate care among IDG members and other healthcare providers to ensure seamless delivery of services.
- Supervise and direct services provided by LPNs, Hospice Aides, and Homemakers.
Requirements:
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in South Carolina.
- 12 months of accumulated experience as a registered nurse within the last 5 years or current clinical knowledge.
- 1 year of hospice care experience.
- CPR certification.
