Overview of the Position

Milestone Centers Inc. is a nonprofit, community-based human services agency that supports individuals with behavioral health challenges and intellectual & developmental disabilities.

The Service Coordinator will work with these individuals to assess their strengths, wants, and needs, and develop individualized service plans to help them achieve their goals within the framework of Service Coordination rules and regulations.

Responsibilities
  • Conduct professional assessments of individuals to identify their strengths, wants, and needs.
  • Develop individualized service plans with identified individuals to help them achieve their goals.
  • Use knowledge of Service Coordination rules, regulations, best practices, and SPA principles to assist individuals with the attainment of service plan goals.
  • Accurately and efficiently document all work performed, including progress notes, individualized service plans, strengths assessments, environmental matrixes, and other collateral documentation as necessary.
  • Abide by all state regulations, CCBHO Performance Standards, and all SPA principles.
