Overview:
Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit organization providing human services to individuals with behavioral health challenges and intellectual disabilities in Western Pennsylvania. We are committed to delivering trauma-informed care and promoting recovery principles.

The Service Coordinator is responsible for assessing the strengths, wants, and needs of clients, developing individualized service plans, and assisting with achieving these goals within the framework of Service Coordination rules and regulations.

Duties include:
  1. Conducting professional assessments of client strengths, wants, and needs through face-to-face, phone, and collateral contacts.
  2. Developing individualized, strengths-based service plans after professional assessment has been completed.
  3. Using knowledge of Service Coordination rules, regulations, best practices, and SPA principles to assist clients with attaining service plan goals.
  4. Accurately documenting all work performed, including progress notes, individualized service plans, and other collateral documentation as necessary.
  5. Abiding by all state regulations, CCBHO Performance Standards, and SPA principles.
  6. Participating in emergency on-call rotation and meeting productivity requirements.
