Care Coordination MSW Job Description

Job Summary:

The Care Coordination MSW Social Worker plays a vital role in our team, working collaboratively with a multidisciplinary team of clinicians to provide high-quality, patient-centered care and address the social determinants of health. This position is responsible for supporting high-cost/high-risk patients, addressing barriers to care, and navigating challenges across the care continuum.

Key Responsibilities:

  • Patient assessment, referrals, and intervention
  • Documentation and maintenance of accurate records
  • Meetings and program development to improve patient outcomes
  • Collaboration with internal and external agencies to ensure seamless care transitions
  • Providing psychosocial assessments, care transitions, and structured support to address social and economic barriers to positive health outcomes
  • Applying best practice interventions based on care standards and referral to services
  • Maintaining best practices, process systems, and key performance metrics to provide effective outcome-based care
  • Conducting psychosocial assessments, social determinants of health screening, and developing plans of care in alignment with individual needs and goals
  • Providing individual telephonic/virtual support and counseling to patients using evidence-based theories and techniques
  • Maintaining accurate and timely referral response, assessment, intervention, and documentation
  • Ensuring ongoing collaboration and communication with the interdisciplinary Care Coordination team, AHS/ACO practices, providers, and care team members
  • Maintaining a current knowledge base of community agencies and key contacts and assisting with patient advocacy, navigation, and engagement with sustainable medical, social, insurance, and benefit systems

Requirements:

  • Master's Degree in Social Work from an accredited school of social work
  • Current LSW license in the State of New Jersey in good standing with the NJ Board of Social Work Examiners
  • Behavioral Health background preferred
  • Strong knowledge of community resources and social service, financial, insurance, and advocacy systems
  • Familiarity with Epic preferred
  • Exceptional organizational and time management skills with the ability to multi-task, problem-solve, and balance and deliver results in a program with multiple projects, programs, and priorities
  • Strong clinical, interpersonal, and analytical skills with clear verbal, telephonic, and written communication
  • Ability to function within an interdisciplinary team and develop and enhance collaborations with local community agencies and organizations
  • Ability to navigate EMR and databases with demonstrated ability, including EMR documentation standards, Microsoft Office suite, and metrics/outcome reporting
  • Strong interpersonal and communication skills, problem-solving, ability to work with diverse populations, compassionate service delivery

Work Environment:

This position is based in Summit, New Jersey, and requires a strong understanding of the local community resources and social service systems. The ideal candidate will have experience in Acute Hospital Case Management or Social Work and be able to ramp up to a full case load within two weeks.

Schedule:

This is a temporary to permanent position with a start date of October 14, 2024, and an end date of January 11, 2025. The schedule is 37.5 hours per week, with 7.5 hours per day, and 5 days per week.
