We are seeking a highly skilled Social Worker to join our team at the VA Northeast Ohio Healthcare System. As a Social Worker in our Community Resource and Referral Center, you will play a critical role in preventing homelessness, promoting mental and physical health stabilization, and ensuring the safety and well-being of our Veterans.
Key Responsibilities- Provide community outreach to Veterans in housing crisis and identify psychosocial needs related to mental health, medical issues, and substance abuse and addiction.
- Conduct psychosocial assessments and develop treatment plans in collaboration with Veterans and interdisciplinary treatment teams.
- Coordinate community-based services, including information and referral for additional services from other VA programs, government programs, and community agency programs.
- Provide crisis intervention services and establish effective therapeutic relationships with Veterans and their families.
- Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
- Licensure or certification in a state to independently practice social work at the master's degree level.
- English language proficiency.
- Experience in community-based social work, accessing local resources, and advocacy with VA and community organizations.
- Working with Veterans who are homeless and at risk of homelessness.
- Direct practice with Veterans with behavioral health and/or substance use disorders.
