Job Title: MSW Social Worker

Job Summary:

We are seeking a highly skilled MSW Social Worker to join our multidisciplinary team in delivering high-quality, patient-centered care. As a key member of our healthcare team, you will address social determinants of health to support safe care transitions and improve outcomes across the facility.

Key Responsibilities:

  • Conduct psychosocial assessments, address barriers to care, and support health and social needs (60% of time)
  • Maintain accurate and timely records of assessments, interventions, and referrals (20% of time)
  • Participate in meetings and contribute to program development (10% of time)
  • Coordinate with internal and external agencies for comprehensive patient support (10% of time)

Job Functions:

  • Support High-Risk Patients: Address care barriers, improve health outcomes, and facilitate resource connections.
  • Provide Psychosocial Support: Assist with social, economic, and behavioral health needs, and guide patients in achieving health goals.
  • Maintain Best Practices: Ensure culturally-sensitive care, equitable access, and reduce health disparities.
  • Conduct Assessments and Develop Plans: Perform screenings, create care plans, and offer telephonic/virtual support.
  • Ensure Effective Collaboration: Communicate with the Care Coordination team and community agencies for comprehensive care.
  • Participate in Meetings: Engage in interdisciplinary forums, training, and provide consultative support for community health workers.

Requirements:

  • Master's degree in social work from an accredited school of social work required
  • 3-5 years in a health-related field, hospital experience preferred
