Role Overview

We are seeking a detail-oriented and compassionate Medical Care Coordinator to join our team at New York Health.

Responsibilities
  • Update and maintain electronic patient records, ensuring accuracy and completeness.
  • Assist in transitioning hard copy records to digital format, streamlining our documentation process.
  • Manage physician inbox and prioritize tasks assigned via ONCO EMR, promoting efficient communication.
  • Provide exceptional patient care by greeting patients, answering phone calls, and addressing front office tasks.
  • Maintain medical inventory, placing orders for new materials as needed, to ensure a well-stocked facility.
  • Record vital signs, weight, and changes in medical history before each appointment, ensuring accurate patient information.
  • Clean and prepare examination rooms prior to patient appointments, maintaining a clean and safe environment.
  • Escort patients to exam rooms, providing a warm and welcoming experience.
  • Record patient interview, history, and chief complaint, facilitating effective treatment plans.
  • Provide patient education on medications, disease management, home treatments, and special diets, empowering patients to take control of their health.
  • Prepare patients for examinations and perform routine screening tests, ensuring a smooth and efficient process.
  • Assist physicians with exams, working collaboratively to provide comprehensive care.
  • Administer injections, phlebotomy, and collect lab specimens, demonstrating expertise in clinical procedures.
Requirements
  • Current NYS LPN license required.
  • BLS certification is mandatory.
  • Consistent professional conduct and meticulous attention to detail are essential.
  • Excellent verbal and written communication skills, as well as excellent interpersonal skills, are necessary for effective collaboration.
  • Experience with EMRs is highly valued.
  • Reliable transportation to any NYH location is required.
