About the Role:As a Registered Nurse, you will be responsible for providing comprehensive skilled care to patients in their homes. You will work autonomously, managing your time efficiently to ensure that patients receive the best possible care. Your duties will include:
- Observing and monitoring patient conditions and performing OASIS assessments to develop individualized care plans
- Administering medication as prescribed by physicians and prioritizing visits for high-risk patients
- Managing multi-disciplinary care and promoting continuity of care with appropriate admissions, transfers, and discharges
- Educating patients and families on disease processes using teach-back methods to ensure understanding
- Completing clinical documentation following agency protocol and Medicare/Federal guidelines
To be successful in this role, you will need:
- A current Registered Nurse license in the state of employment
- Minimum one year of experience as an RN in an acute care setting, with home care experience preferred
- A valid driver's license and auto insurance
- The ability to meet the physical demands of the job
