  • Observing and monitoring patient conditions and performing OASIS assessments to develop individualized care plans
  • Administering medication as prescribed by physicians and prioritizing visits for high-risk patients
  • Managing multi-disciplinary care and promoting continuity of care with appropriate admissions, transfers, and discharges
  • Educating patients and families on disease processes using teach-back methods to ensure understanding
  • Completing clinical documentation following agency protocol and Medicare/Federal guidelines
Requirements:

To be successful in this role, you will need:

  • A current Registered Nurse license in the state of employment
  • Minimum one year of experience as an RN in an acute care setting, with home care experience preferred
  • A valid driver's license and auto insurance
  • The ability to meet the physical demands of the job
