We are seeking a skilled Care Navigator to join our team at Phoenix Children's Hospital. Key Responsibilities
• Provide care management support to patients and families, including verifying insurance eligibility, demographic information, and coordinating communication.
• Execute administrative portions of care coordination under the direction of a licensed healthcare professional and in cooperation with the healthcare multidisciplinary team.
• Obtain medical records, provide patient education, and facilitate phone/fax/email communication as indicated to expedite tasks.
• Log all encounters with and about patients in the electronic medical record and escalate barriers to implementing the care plan to the licensed healthcare provider and healthcare team.
Program and Population Support
• Manage data collection and entry, maintain databases, and coordinate new patient referrals.
• Assist with marketing program services and recruitment, and assist with administration of survey tools to patients/families, providers, and care management associates.
Customer Service
• Communicate clearly in both written and verbal communications with all internal and external customers, providing excellent service routinely in interactions with all internal and external customers.
• Collaborate with others to acquire information and materials, answer telephone calls, take messages, maintain schedules, and respond to email within 24 hours or one business day.
Education and Training
• Participate in a variety of educational programs to maintain current skills and competency levels.
