This role is pivotal in overseeing the Medication Clinic operations. You will act as a case manager for clients utilizing the medication clinic services. Your responsibilities will include evaluating physical health requirements, facilitating referrals to local healthcare providers, and collaborating with psychiatrists for medication management and administration. Additionally, you will provide a comprehensive suite of treatment, rehabilitation, and support services for individuals engaged in Assertive Community Treatment.

This position is part-time, requiring 20 hours of work each week, with flexible scheduling available during weekdays.

KEY RESPONSIBILITIES


• Organize and manage the medication clinic

• Educate clients on medication usage through detailed review sessions

• Serve as a case manager for clients requiring medication refills

• Maintain accurate documentation related to medication management and psychiatric consultations

• Administer intramuscular medications, distribute sample medications, and keep precise records of all medications handled

• Assist in completing prior authorizations for medications

• Keep an updated list of injections, coordinate their ordering and delivery, verify current orders, and monitor for any side effects

• Measure vital signs and oversee the prediabetes screening process

• Utilize computer systems effectively for documentation and communication

• Under the guidance of the team psychiatrist, manage the medication delivery system and ensure integration into clients' personalized treatment plans. Take a proactive role in medication administration and medical services

• Provide education and support regarding medications, mental health conditions, and co-occurring health issues to clients and the ACT team

• Participate in daily staff meetings and treatment planning reviews, along with weekly supervision sessions

• Complete necessary Authorizations of Disclosure

• Collaborate with the ACT team on client health and medication matters

• Ensure proper storage and administration of medications in compliance with established policies

• Adhere to safety protocols, including the use of protective equipment and proper waste disposal

• Comply with regulatory standards such as CLIA and OSHA

• Perform additional duties as assigned
