Licensed Practical Nurse for Community Care

We are excited to announce a $2,000 sign-on bonus as we expand our services across various clinics.
As a vital member of our team, you will oversee the Medication Clinic operations. Your role will involve acting as a case manager for clients utilizing our medication services. You will assess physical health requirements, refer clients to community healthcare providers, and collaborate with psychiatrists to manage and administer medications. Additionally, you will provide a comprehensive array of treatment, rehabilitation, and support services to individuals enrolled in Assertive Community Treatment.
This position is part-time, requiring 20 hours of work per week, scheduled between 8:00 AM and 5:00 PM on select weekdays, Monday through Friday.
KEY RESPONSIBILITIES
• Organize and coordinate the operations of the medication clinic
• Review and discuss medication education materials with clients
• Serve as a case manager for clients requiring medication refills
• Maintain accurate documentation related to the medication clinic and psychiatric consultations
• Administer intramuscular medications, manage sample medications, and keep precise logs of all medications received and dispensed
• Assist in completing prior authorizations
• Keep an updated list of injections, coordinate orders and deliveries, verify current orders, and monitor for any side effects
• Measure vital signs and facilitate prediabetes screening
• Demonstrate proficiency with computer systems
• Under the guidance of the team psychiatrist, manage the medication delivery system and integrate medication administration into clients' personalized treatment plans. Lead or participate in providing medication administration and medical services
• Educate, train, and support clients and the ACT team on medications, mental health conditions, and co-occurring health issues
• Attend daily organizational meetings and treatment planning reviews, along with weekly supervision sessions
• Complete necessary Authorizations of Disclosure
• Collaborate with the ACT team regarding client health and medication concerns
• Ensure proper storage and administration of medications (including injections) in compliance with Medication Dispensation, Sample, and Patient Assistance Program Policies
• Adhere to standard precautions, including the use of protective barriers and proper disposal of infectious waste
• Comply with CLIA and OSHA regulations
• Perform other assigned duties as necessary
