In this role, you will oversee the Medication Clinic's operations. You will act as a case manager for clients utilizing the medication clinic services. Your responsibilities will include evaluating physical health requirements, facilitating referrals to local healthcare providers, and managing medication alongside the psychiatrist. You will also deliver a variety of treatment, rehabilitation, and support services for individuals engaged in Assertive Community Treatment.

This position is part-time, requiring 20 hours of work per week, scheduled between 8:00 am and 5:00 pm on selected weekdays, Monday through Friday.

KEY RESPONSIBILITIES


• Organize and coordinate the medication clinic

• Review medication information with clients

• Serve as a case manager for clients requiring medication refills

• Maintain accurate documentation related to the medication clinic and psychiatric consultations

• Administer intramuscular medications, dispense sample medications, and keep precise logs of all medications received and distributed

• Assist in completing prior authorizations

• Manage the list of injections, oversee ordering and delivery, verify orders, and discuss any potential side effects

• Measure vital signs and coordinate prediabetes screening processes

• Possess a strong understanding of computer systems

• Under the guidance of the team psychiatrist, organize and manage the medication delivery system, ensuring integration with clients' treatment plans. Take a lead role or participate in medication administration and medical services

• Provide education, training, and support regarding medications, mental health conditions, and co-occurring health issues to clients and the ACT team

• Attend daily staff meetings and treatment planning reviews, as well as weekly supervision sessions

• Complete necessary Authorizations of Disclosure

• Collaborate with the ACT team on client health and medication concerns

• Ensure proper storage and administration of medications (including injections) in compliance with relevant policies

• Adhere to standard safety precautions, including the use of protective barriers and proper disposal of hazardous waste

• Follow all applicable regulations

• Perform additional duties as assigned
