Job Summary

We are seeking a highly skilled Clinical Care Coordinator to join our team at Aylo Health. As a key member of our Care Team, you will be responsible for managing encounters, clinical relationships, and patient experiences.

Key Responsibilities
  • Initiate outgoing calls to patients with pending post-visit care
  • Manage patient care and provider relationships
  • Manage turnaround time for encounters
  • Serve as a point of escalation for a Level I Clinical MA
  • Receive and process e-prescription requests from pharmacies
  • Call-in and submit patient medication to pharmacies as instructed by providers
  • Contact insurance companies to receive prior authorization for medication
  • Correspond with providers through electronic medical records regarding patient activity
  • Answer patient inquiries regarding general medical questions
  • Obtain necessary personal and clinical information from patients
  • Accurately document telephone encounters
  • Receive test results from providers through telephone encounters
  • Contact patients by phone in a timely manner, ensuring test results are conveyed accurately and privately following all HIPAA regulations
Qualifications
  • High School graduate or equivalent education
  • Proven history of driving a positive patient experience
  • For MAs - Completion of an accredited Medical Assistant Program with a current Medical Assistant Certification/Registration
  • For LPNs - Completion of an accredited LPN program
  • 5+ years' work experience in a clinical private practice setting with EMR experience
  • Ability to work quickly and efficiently to initiate high volumes of outgoing calls
  • Demonstrated ability to organize, coordinate, prioritize, and facilitate many ongoing tasks
  • Professional verbal and written communication skills
  • Ability to work independently or with a team
  • Professional appearance and positive attitude
