Job Summary

Adoration Home Health and Hospice is seeking a skilled LPN to provide high-quality care to our patients in the comfort of their own homes. As a Home Health LPN, you will work under the supervision of a Registered Nurse to deliver routine care, monitor patient health status, and assist with treatments. Your excellent communication skills and ability to work independently will enable you to build strong relationships with patients and their families.

Responsibilities
  • Provide one-on-one care to patients, following their individualized plan of care and promoting their dignity and independence.
  • Monitor patients' vital signs and report any abnormal findings to the Case Manager and Physician.
  • Administer medications, feedings, oxygen, and ostomy care as needed, and document the results of these interventions.
  • Complete routines established by other care professionals, such as PT, OT, and Speech Therapists.
  • Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections.
  • Accurately document all nursing actions, communication with family, case manager, and physicians, and document progress and outcomes for established goals.
  • Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
  • Ensure all patient needs are met in accordance with the instructions of the physician.
  • Monitor incidents involving individuals and ensure incident reports are completed timely and accurately.
  • Ensure all medical equipment is maintained and report faulty equipment to DME for repair.

Qualifications
  • LPN/LPT/LVN degree from an accredited college of nursing with current unrestricted registration and license in the state of practice.
  • Valid driver's license, acceptable driving record, and proof of car insurance.
  • CPR professional certification.
  • Experience in administering medications.
  • 1+ years of nursing experience preferred.
  • Home health care experience preferred.
  • Able to lift and carry items weighing up to 20 pounds.
  • Able to bend, stoop, push, pull, sit, and walk for periods of time.
  • Must be able to communicate both verbally and in writing.
