Overview
All Ways Caring HomeCare is seeking a compassionate and skilled Nursing Professional - Intake Specialist to join our team of dedicated clinicians. As a key member of our nursing group, you will focus on delivering exceptional patient care in a variety of settings, including Home Health, Hospice, Residential, and Private Duty.

Key Responsibilities

  • Provide one-on-one care to clients, adhering to their individualized plans of care and promoting their dignity and independence.
  • Monitor clients' vital statistics, reporting any abnormal findings to the case manager and physician for assessment.
  • Administer medications, feedings, oxygen, and ostomy care as needed, documenting the results of these interventions.
  • Collaborate with other care professionals, such as Physical Therapists, Occupational Therapists, and Speech Therapists, to ensure comprehensive care.
  • Implement and maintain infection control policies, reporting any possible communicable diseases or infections to the appropriate authorities.
  • Accurately document all nursing actions, communication with family members, case managers, and physicians, as well as progress and outcomes for established goals.
  • Order supplies, medications, and feedings as needed to ensure adequate stock levels.
  • Ensure all patient needs are met in accordance with physician instructions.
  • Monitor incidents involving individuals, completing incident reports in a timely and accurate manner, and informing case managers of any necessary actions.
  • Maintain and report faulty medical equipment to the Durable Medical Equipment department for repair.

Requirements

  • Current licensure as a LPN/LVN/LPT
  • Experience in Home Health and Hospice care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
