OptumCare Oregon is in search of a full-time Nurse Practitioner to deliver clinical services across diverse environments and tackle the care gaps often overlooked in conventional clinic appointments.


Key Responsibilities:

  • Document and manage chronic health issues to enhance patient records.
  • Address quality care deficiencies.
  • Identify and tackle factors related to chronic conditions that may have gone unnoticed, collaborating with patients to achieve optimal health outcomes.
  • Inform patients about available resources within the OptumCare Network.
  • Connect patients to care management or additional services when necessary.


We seek clinicians who prioritize patient care over patient volume. Our ideal candidates will take the necessary time to genuinely address the needs of our patients. We are looking for professionals who are adaptable to various settings (including home visits), capable of building rapport with patients, and willing to embrace flexibility. This role does not entail managing a dedicated panel of patients or ongoing therapeutic management of acute or chronic conditions. Care locations may include:

  • Primary care clinics affiliated with the OptumCare Network.
  • Various specialty clinics (specific locations will be determined based on clinician's home base).
  • Home visits and long-term care facilities.
  • Virtual consultations.

Position Highlights:

  • Full-time role, 5 days per week.
  • Travel required to various clinic sites, skilled nursing facilities, adult family homes, and occasional home visits.
  • Manage approximately 10-12 patients daily through various modalities (in-person and telemedicine visits) while in the clinic.
  • Conduct 6-7 patient visits per day during home visits.
  • Ensure that problem lists accurately reflect chronic health conditions.
  • Facilitate patient engagement with their primary care providers.
  • Educate patients about available resources through OptumCare.

Required Qualifications:

  • Active, unrestricted Nurse Practitioner license in Oregon and/or Washington or the ability to obtain it by the start date. Licensure must be unencumbered, free of any open or unresolved disciplinary actions, including probation or restrictions against the privilege to practice.
  • Current certification as a Nurse Practitioner.


Preferred Qualifications:

  • Current DEA certificate prior to start date.
  • Experience or training in gerontology, geriatric medicine, and/or chronic care.


Washington Residents Only: The salary range for this role is $88,000 to $173,200 annually. Compensation is based on various factors, including local labor markets, education, work experience, and certifications. UnitedHealth Group adheres to all applicable minimum wage laws.
