About the Role

We are seeking a dedicated Care Specialist to join our team in Contra Costa and Alameda Counties. As an Occupational Therapist, you will work closely with physicians to develop personalized treatment plans for our clients.

Responsibilities
  • Assess the home environment to identify potential fall risks and develop strategies to improve client safety.
  • Work cooperatively with physicians to create tailored treatment plans based on each client's specific needs.
  • Identify equipment needs and develop adaptive plans to enhance client independence and functionality.
  • Report client progress and changes in condition to the Director of Nursing and Physician.
  • Prepare clinical summaries, progress reports, and discharge planning documents for submission to the Director of Nursing.
Requirements
  • Graduation from an occupational therapy program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) or its predecessor organization.
  • A minimum of two years' experience as an Occupational Therapist, with licensure in the State of California.
  • Adherence to the Occupational Therapy Code of Ethics and knowledge of AOTA standards.
