Job Overview

The Licensed Practical Nurse (LPN) is responsible for providing direct patient care, including conducting laboratory tests, administering treatments, and managing medication delivery.

This role operates under the supervision of the Nurse Manager, RN Supervisor, RN, and/or physician to prepare patients for examinations and treatments.

The LPN will engage in clinical tasks, quality assurance activities, and performance enhancement initiatives in support of the healthcare practice as assigned.

Key Responsibilities

1. Deliver both direct and indirect patient care in alignment with the LPN Practice Act, organizational policies, and professional standards.

2. Gather patient health information, including vital signs, weight, primary concerns, and pain evaluations, while considering the patient's physical, psychological, cultural, and spiritual needs.

Involve patients and their families, along with other healthcare professionals, when necessary.

3. Execute patient care treatments and tests as prescribed by the physician.

4. Administer medications and immunizations using appropriate methods tailored to the patient's age.

5. Ensure documentation is complete, accurate, and timely.

6. Provide education and guidance to patients and families within the scope of practice.

7. Assist in evaluating the educational needs of patients and identify potential barriers to learning.

8. Supply educational materials and instructions based on assessed needs.

9. Document all educational interactions, including topics discussed and patient understanding.

10. Foster collaborative relationships with physicians to achieve optimal patient outcomes.

11. Prepare patients for examinations, treatments, and procedures.

12. Assist physicians with treatments and procedures as needed.

13. Ensure patient safety in accordance with healthcare policies.

14. Utilize two forms of patient identification before initiating any care.

15. Label all patient specimens accurately as per policy.

16. Maintain compliance with all regulatory requirements.

17. Conduct quality control testing for point-of-care testing as per policy.

18. Participate in non-punitive medication reporting.

19. Adhere to infection control protocols to minimize the risk of infection transmission.

20. Demonstrate leadership qualities inherent to professional nursing practice.

21. Solve problems independently while seeking assistance from supervisors when necessary.

22. Maintain accountability for actions taken.

23. Delegate patient care responsibilities appropriately.

24. Act as a resource for colleagues.

25. Advocate for patients' needs.

26. Serve as a mentor for new hires and students.

27. Contribute to reducing departmental costs.

28. Support compliance with regulatory standards.

29. Engage in continuous readiness for inspections and surveys.

30. Coordinate communication between patients, families, healthcare providers, pharmacies, specialists, and insurance companies.

31. Provide guidance based on protocols and physician directives, directing patients to appropriate care levels.

32. Facilitate access to prescription refills as per physician orders.

33. Assist with prior authorizations and indigent programs based on the patient's financial situation or insurance coverage.

Support performance improvement initiatives within the healthcare practice.

34. Assist in data collection as directed by management.

35. Integrate performance improvement initiatives into daily practice.

36. Maintain a safe and clean care environment.

37. Ensure daily replenishment of supplies in patient care areas.

38. Perform point-of-care testing as ordered by the physician.

39. Complete all quality control testing as per policy.

40. Identify and follow up on results that fall outside acceptable ranges before conducting patient testing.

41. Record patient test results accurately on the appropriate forms.

42. Prepare forms, copy records, and respond to requests as per policies.

43. Assist in maintaining patient medical records for efficient information retrieval.

44. Participate in various office functions as necessary.

45. Perform additional duties as required.

Qualifications

1. Graduate from an accredited Practical Nursing program.

2. Strong leadership, organizational, independent thinking, and critical judgment skills, along with knowledge of nursing legalities.

3. Excellent communication skills for conveying complex topics to diverse audiences.

Preferred Qualifications

1. Previous experience as an LPN.

Licensure, Certification & Clearances

1. Current licensure as a Licensed Practical Nurse in Pennsylvania.

2. Current Basic Life Support (BLS) certification from an approved provider.

3. Act 33, Act 34, and Act 73 clearances.

Supervisory Responsibilities

This position does not have direct supervisory responsibilities but serves as a coach and mentor for other department roles.

Work Schedule

The incumbent will be scheduled based on operational needs, including rotating shifts and on-call duties.
