Job Summary

We are seeking a highly skilled and experienced Quality Improvement Director to join our team at HeartShare Human Service of New York. The successful candidate will be responsible for maintaining an annual quality improvement calendar, conducting quarterly case record reviews, and analyzing data to provide detailed reports.

Key Responsibilities
  • Maintain an annual quality improvement calendar to schedule and conduct quarterly case record reviews, utilization reviews, consumer satisfaction surveys, and facility safety reviews.
  • Design and maintain agency-wide dashboards, including monthly metrics and measurement against program-specific outcomes.
  • Analyze and interpret outcome and performance measurement reports from state oversight agencies.
  • Design and implement training programs, workshops, and presentations related to quality improvement and program evaluation.
  • Conduct literature reviews to identify best practices in quality improvement and program evaluation.
  • Participate in relevant committees, such as corporate compliance, risk management, and DDQI.
  • Assist the Senior Vice President of Business Analytics and Quality with creation of quarterly board reports and coordination of annual trainings.
  • Represent the organization on various provider organization workgroups and committees.
Requirements
  • Master's degree in social work, psychology, sociology, or closely related field of study.
  • 12-months experience in social work, human service, community service management, health education, quality improvement, or closely related field.
