Patient Care Coordinator

The Patient Care Coordinator is responsible for conducting thorough patient interviews and evaluations, assessing the appropriateness of care levels, as well as identifying risks and strengths. This role involves engaging patients in their treatment processes, including ongoing care planning and discharge strategies. A solid understanding of community resources and referral systems is essential, along with the ability to collaborate effectively with community-based providers and organizations. Knowledge of addiction and mental health recovery processes, services, and support systems is also required. The Patient Care Coordinator will foster recovery through positive role-modeling and respectful interactions with both patients and colleagues.
KEY RESPONSIBILITIES:- Conduct daily assessments with individual caseloads to evaluate ongoing needs and treatment progress, documenting daily contact notes.
- Perform comprehensive biopsychosocial and Columbia assessments within 48 hours of patient admission.
- Develop treatment and discharge plans in partnership with patients and their identified support systems.
- Facilitate assigned group sessions.
- Engage in staff supervision, in-service training, and continuing education necessary for maintaining certification.
- Manage caseload effectively and track multiple required tasks utilizing Electronic Health Records.
- Maintain regular communication with referral sources and advocacy organizations.
- Participate in necessary continuing education for certification compliance.
- Interact with financial and utilization management teams for reimbursement purposes under supervision.
- Complete all required documentation in line with organizational policies and regulatory standards.
- Adhere to the professional code of ethics, including confidentiality, boundaries, dual relationships, and professional conduct as a representative of Gosnold.
- Demonstrate strong customer service skills.
- Perform additional duties as assigned.
MINIMUM QUALIFICATIONS:
- Education: A Bachelor's degree in a healthcare or related field is preferred.
- Experience: A minimum of one (1) year of experience in mental health, addiction, or home health/hospice settings is required. Experience in a multidisciplinary team treatment approach is also essential.
- Skills: Exceptional customer service, along with strong verbal and written communication abilities. Proficiency in technology and computer literacy is necessary.
