Job Title: Registered Nurse - Post-Acute Care Specialist

About the Role:
  • Provide high-quality nursing care to patients in post-acute settings, including skilled nursing facilities and assisted living facilities.
  • Administer individualized treatments and medications to residents, ensuring accurate and timely delivery.
  • Develop and implement care plans in conjunction with residents and medical teams, prioritizing patient-centered care.
  • Set up and monitor medical equipment, ensuring proper use and maintenance.
  • Communicate effectively with residents, families, and healthcare staff, addressing concerns and providing education on care options.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination.
  • Document interventions and medical condition histories accurately, maintaining up-to-date records.
  • Participate in investigations into incidents, ensuring thorough and accurate reporting.
Requirements:
  • Current Massachusetts state license to practice as a Registered Nurse (RN).
  • Completion of an approved nursing program.
  • Experience in post-acute care or in-patient hospital settings (at least 4 months).
  • Excellent organizational skills and ability to prioritize tasks effectively.
