Job Summary

Neighborhood Healthcare is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a critical role in providing clinical pharmacy services to support our chronic disease management programs.

Key Responsibilities
  • Manage drug regimens to ensure accuracy and compliance with practice agreements and authorized prescribers.
  • Work with patients to ensure selection of appropriate drugs, including monitoring progress and consulting on effects.
  • Communicate with patients regarding medication use and answer questions.
  • Advises and reviews prescriptions issued by physicians or other authorized prescribers to ensure accuracy.
  • Perform patient assessments, modify, add, discontinue drug therapy, and order related laboratory tests to monitor drug therapy per collaborative practice agreement.
  • Ensure pharmacotherapy treatment recommendations in the disease management program are consistent with formulary initiatives.
  • Perform literature searches and analyze data to answer questions surrounding drug therapy.
  • Provide support to providers and clinic staff according to standard pharmacy practices.
  • Function at highest level according to credentials and competencies.
  • Share accountability for overall patient health outcomes and work in coordination with care teams.
  • Operate to instill confidence in our care and facilities to patients, fellow employees, and other stakeholders.
  • Contribute to the success of the organization by participating in quality improvement activities.
  • Respond promptly and with caring actions to patients and employees.
  • Maintain professional working relationships with all levels of staff, patients, and the public.
  • Cooperate in accomplishing department goals and objectives.
Requirements
  • Doctor of Pharmacy (PharmD) degree required.
  • Completion of Pharmacy Practice Residency Program required.
  • Valid CA Pharmacist license.
  • Board Certified Ambulatory Care Pharmacist (BCACP) certification preferred.
  • Two years' experience within ambulatory care setting or other relevant residency training.
  • One year experience managing patients with diabetes, cholesterol, hypertension, nicotine cessation, and/or asthma/COPD preferred.
Additional Qualifications
  • Excellent verbal and written communication skills, including superior composition, typing, and proofreading skills.
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form.
  • Knowledgeable about and experience with federal and state pharmacy laws.
  • Knowledgeable about and experience with standard pharmacy practices.
  • Ability to successfully manage multiple tasks simultaneously.
  • Excellent planning and organizational ability.
  • Ability to work as part of a team as well as independently.
  • Ability to work with highly confidential information in a professional and ethical manner.

Neighborhood Healthcare requires employees to be fully vaccinated (including a booster dose) against COVID-19 and demonstrate proof of vaccination/booster upon hire or receive approval for an exemption based on a medical reason, disability, or religious belief. Proof of booster dose will be contingent upon booster eligibility.

Pay Range: $140,000 to $160,000 per year, Depends on Experience (DOE)
