The Clinical Pharmacist works to support the mission and vision of Neighborhood Healthcare by providing excellent patient-centered care and services within a dynamic refill team environment. This role will utilize pharmacist skills such as assessment, critical thinking, judgement, decision making in reviewing and filling patient prescriptions. Additionally, this role will provide cross-cover support in the retail pharmacy, including site and personnel oversight.Essential duties and responsibilitiesExamines problem lists and medication lists to determine if there are contraindications or significant interactions, and make appropriate recommendations for changeSubstitutes therapeutically equivalent medications in accordance with health plan formularies independentlyClarifies drug interactions, directions for use, and substitutions in concert with external pharmacists, and notifies provider when appropriateAnswers clinical questions from providers accurately, promptly, and thoroughly using the most up to date clinical guidelines and evidenceProvides retail pharmacy support and oversight, as neededProvides clinical oversight and direction of non-pharmacist staff, as neededAttends and participates required meetings and trainingsPerforms all job functions with Neighborhood's mission, vision, values, and strategic plan in mindLearns and navigates Neighborhoods electronic medical record (EMR) to investigate refill requests initiated by patients and pharmaciesCommunicates via the EMR with other Neighborhood staff as it relates to patient care in accordance with the Neighborhood Pharmacist-Nurse Refill ProtocolRefills medications on behalf of providers according to Neighborhood's Pharmacist-Nurse Refill ProtocolOrders labs needed to monitor medications with the Neighborhood Pharmacist-Nurse Refill ProtocolOrders outstanding routine labs required for health maintenanceReviews telephone encounters to identify duplicate or missing medications on patient medication listsContacts patients and pharmacies to clarify medication discrepancies prior to refilling a medicationMaintains accurate and complete records in the electronic health records of all patient encountersQuality Management:Contributes to the success of the organization by participating in quality improvement activitiesCooperates in accomplishing department goals and objectivesCustomer Relations:Responds promptly and with caring actions to patients and employeesMaintains professional working relationship with all levels of staff, patients, and the publicEDUCATION/EXPERIENCEPharmacy doctorate degree from an ACPE accredited pharmacy school requiredValid CA pharmacist license requiredBoard Certification Pharmacotherapy Specialist or other specialty preferredCurrent BLS certification requiredPharmacy practice residency preferredBilingual (English/Spanish) or (English/Arabic) preferredADDITIONAL QUALIFICATIONS (Knowledge, Skills, and Abilities)Excellent verbal and written communication skills, including superior composition, typing, and proofreading skillsAbility to interpret a variety of instructions in written, oral, diagram, or schedule formKnowledgeable about and experience with pharmacy practices and protocolsAbility to successfully manage multiple tasks simultaneouslyExcellent planning and organizational abilityAbility to work as part of a team as well as independentlyAbility to work with highly confidential information in a professional and ethical mannerPhysical RequirementsAbility to lift/carry 20 lbs/weightAbility to stand for long periods of timeCOMPLIANCE (Safety and HIPPA)Maintain current knowledge of policies and procedures as they relate to safe work practicesFollow the NHC Pharmacist-Nurse Refill Protocol for refill authorizations Follows all safety procedures as outlined in Neighborhood Healthcare's Illness and Injury Prevention Plan (IIPP) and report any injuries and/or unsafe conditions immediately Maintains current knowledge of policies and procedures as they relate to safe work practices Follows all safety procedures and report unsafe conditions Uses appropriate body mechanics to ensure an injury free environment Familiarity with location of nearest fire extinguisher and emergency exits Follows all infection control procedures including blood-borne pathogen protocols Maintains privacy of all patients, employee and volunteer information and access such information only on a need-to-know basis for business purposes Complies with all regulations regarding corporate integrity and security obligations Reports all behavior and/or activity that are unethical, fraudulent, or unlawfulPay Range: $140,000 to $160,000 per year, Depends on Experience (DOE)
