The Clinical Pharmacist will work directly with Neighborhood Healthcare patients to provide clinical pharmacy services to support Neighborhood’s chronic disease management programs. This role will manage drug regimens to ensure in accuracy and compliance with practice agreements and authorized prescribers.

Essential Duties & Responsibilities

  • Participates in all department clinical programs, projects, and daily activities with pharmaceutical care and related issues
  • Acts as a professional and non-professional consultant to clinic staff on medications
  • Works with patients to ensure selection of appropriate drugs, including monitoring the patient’s progress, consulting with the patient on the effects of the drugs, and ensuring patients follow the prescribed drug regimens
  • Communicates with patients regarding the appropriate use of medications and answers patient questions
  • Advises and reviews prescriptions issued by physicians or other authorized prescribers to ensure accuracy
  • Performs patient assessments, modifies, adds, discontinues drug therapy, and orders related laboratory test to monitor drug therapy per collaborative practice agreement
  • Ensures that pharmacotherapy treatment recommendations in the disease management program are consistent with formulary initiatives
  • Performs literature searches and analyzes data to answer questions surrounding drug therapy
  • Provide the appropriate support to the providers and clinic staff according to standard pharmacy practices
  • Functions at highest level according to credentials and competencies
  • Shares accountability for overall patient health outcomes and working in coordination with care teams
  • Operates to instill confidence in our care and in our facilities to patients, fellow employees, and other stakeholders
  • Contributes to the success of the organization by participating in quality improvement activities
  • Responds promptly and with caring actions to patients and employees
  • Maintains professional working relationships with all levels of staff, patients, and the public
  • Cooperates in accomplishing department goals and objectives
Requirements
  • Doctor of Pharmacy (PharmD) degree required
  • Completion of Pharmacy Practice Residency Program required
  • Valid CA Pharmacist license
  • Board Certified Ambulatory Care Pharmacist (BCACP) certification preferred
  • Two years’ experience within ambulatory care setting or other relevant residency training
  • One year experience managing patient with diabetes, cholesterol, hypertension, nicotine cessation, and/or asthma/COPD preferred
Additional Qualifications (Knowledge, Skills and Abilities)
  • Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
  • Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
  • Knowledgeable about and experience with federal and state pharmacy laws
  • Knowledgeable about and experience with standard pharmacy practices
  • Ability to successfully manage multiple tasks simultaneously
  • Excellent planning and organizational ability
  • Ability to work as part of a team as well as independently
  • Ability to work with highly confidential information in a professional and ethical manner


Neighborhood Healthcare requires employees to be fully vaccinated (including a booster dose) against COVID-19 and demonstrate proof of vaccination/booster upon hire or receive approval for an exemption based on a medical reason, disability or religious belief. Proof of booster dose will be contingent upon booster eligibility.

Pay Range: $140,000 to $160,000 per year, Depends on Experience (DOE)
