Job Summary

We're seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you'll work directly with our patients to provide clinical pharmacy services that support our chronic disease management programs. Your expertise will help ensure accurate and compliant drug regimens, and you'll work closely with our team to provide exceptional patient care.

Key Responsibilities
  • Participate in department clinical programs, projects, and daily activities related to pharmaceutical care and issues
  • Act as a professional and non-professional consultant to clinic staff on medications
  • Work with patients to ensure selection of appropriate drugs, monitor progress, and ensure compliance with prescribed regimens
  • Communicate with patients regarding medication use and answer questions
  • Advises and reviews prescriptions issued by physicians or other authorized prescribers to ensure accuracy
  • Perform patient assessments, modify, add, or discontinue drug therapy, and order related laboratory tests
  • Ensure pharmacotherapy treatment recommendations align with formulary initiatives
  • Perform literature searches and analyze data to answer questions surrounding drug therapy
  • Provide support to providers and clinic staff according to standard pharmacy practices
Requirements
  • Doctor of Pharmacy (PharmD) degree required
  • Completion of Pharmacy Practice Residency Program required
  • Valid CA Pharmacist license
  • Board Certified Ambulatory Care Pharmacist (BCACP) certification preferred
  • Two years' experience in an ambulatory care setting or relevant residency training
  • One year experience managing patients with diabetes, cholesterol, hypertension, nicotine cessation, and/or asthma/COPD preferred
