Job Title: Ambulatory Care Nurse

We are seeking a skilled Ambulatory Care Nurse to join our team at Mass General Brigham. As an Ambulatory Care Nurse, you will be responsible for providing high-quality patient care in an outpatient setting.

Job Summary:

The Ambulatory Care Nurse will work collaboratively with a multidisciplinary team to provide comprehensive care to patients with various medical conditions. This includes assessing patients, developing and implementing individualized care plans, and educating patients and families on disease management and self-care.

Responsibilities:
  • Provide direct patient care in an ambulatory setting, including assessing patients, developing and implementing care plans, and educating patients and families.
  • Collaborate with a multidisciplinary team to provide comprehensive care to patients with various medical conditions.
  • Develop and maintain accurate and detailed patient records, including medical histories, treatment plans, and progress notes.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in ambulatory care.
Requirements:
  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • Minimum of 1 year of clinical nursing experience in an ambulatory clinic or practice setting.
  • Experience with telephone triage and telephone-based clinical assessment techniques.
  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with patients, families, and healthcare teams.
