Job Summary:
The Bilingual LTSS Service Coordinator plays a crucial role in delivering service coordination for a designated group of individuals within specialized programs. This role is primarily field-based, allowing for remote work while also necessitating in-person meetings with patients and providers.

The coordinator will collaborate closely with individuals to facilitate the Person-Centered Planning process, ensuring that each member's preferences, needs, and self-identified goals are thoroughly documented. Responsibilities include conducting assessments, developing comprehensive Person-Centered Support Plans (PCSP), and engaging with Medical Directors and interdisciplinary teams to create integrated care plans that address both physical and behavioral health needs.


Key Responsibilities:

1. Conduct face-to-face program assessments utilizing established tools and motivational interviewing techniques to evaluate and manage individual waiver needs, including LTSS, behavioral health, and physical health requirements.

2. Identify members with potential clinical healthcare needs and coordinate their cases with clinical healthcare management and interdisciplinary teams to ensure effective care coordination.

3. Manage the non-clinical needs of members with chronic conditions, ensuring the efficient use of long-term services and supports.

4. Document short and long-term service goals in collaboration with the member's care team, which may include caregivers, family, and service providers.

5. Mentor and train new staff, serving as a subject matter expert and contributing to process improvement initiatives.

6. Submit utilization and authorization requests to management, ensuring alignment with individual care plans.

7. Report critical incidents to relevant internal and external parties, including state and county agencies, and assist in appeals and audits.


Qualifications:

Requires a BA/BS degree and a minimum of 2 years of experience in a social work setting, or a combination of education and experience that provides an equivalent background. Specific qualifications may vary based on state laws and contract requirements.


Preferred Skills:

1. Degree in a healthcare-related field is preferred.

2. Bilingual proficiency in English and Spanish is strongly preferred.
