**Responsibilities:**
* Coordinate intake processes, conducting initial assessments and developing case plans to meet client needs
* Connect clients with essential resources, including job training programs, housing assistance, and mental health services
* Facilitate regular check-ins with clients to monitor progress and adjust plans as necessary
* Collaborate with internal and external stakeholders to ensure seamless care coordination
* Document all services and interactions with clients per state regulations

**Annual Salary:** $58,000 - $68,000 per annum

**Qualifications:**
* Master's degree in social work, counseling, or a related field
* At least two years of experience in case management, social work, or a related field
* Valid driver's license

**Work Environment:**
