Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Uptown Healthcare Center in Denver, CO. As an LPN, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to ensure the best possible outcomes for our residents.

Key Responsibilities

• Assess patients by physical examination, including pertinent diagnostic testing to determine health status.
• Administer medications and treatments as ordered by physicians.
• Participate in the care planning process to develop and implement individualized care plans for residents.
• Supervise LPNs and certified nursing assistants to ensure the delivery of high-quality patient care.
• Communicate effectively with physicians, family members, and other healthcare professionals to ensure seamless care coordination.

Requirements

• Graduate of an accredited school of nursing with a current, unencumbered LPN license in Colorado.
• One year of experience in a long-term care environment preferred.
• Knowledge of computer systems, including Point Click Care, and nursing and medical practices, procedures, and standards of care.
• Current CPR certification.
