Job Title: LPN - Licensed Practical Nurse

At Sunrise Senior Living, we are committed to providing high-quality care to our residents. We are seeking a skilled and compassionate LPN to join our team as a Wellness Nurse.

Responsibilities:
  • Conduct regular wellness visits for residents to assess their health and well-being.
  • Develop and implement individualized care plans to meet the unique needs of each resident.
  • Collaborate with the Resident Care Director and other healthcare professionals to ensure seamless care delivery.
  • Monitor and report any changes in resident health status to the Resident Care Director and other relevant parties.
  • Provide education and support to residents and their families on health-related topics.
  • Participate in quality improvement initiatives to enhance the overall care experience.
Requirements:
  • Graduate of an accredited LPN program.
  • Current state license as a Licensed Practical Nurse.
  • Minimum of one year experience in a healthcare setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficiency in electronic medical records and other healthcare software.
