We are seeking a dedicated and compassionate Physical Therapist to join our team of skilled home health care providers. As a Physical Therapist at Pinnacle Home Care Inc., you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

The Physical Therapist is a professional member of the healthcare team who provides Physical Therapy services to patients in compliance with all applicable practice acts. The Physical Therapist performs duties within standards of clinical practice as defined by the Department of Professional Regulation, the American Physical Therapy Association, and the policies and procedures of the agency.

Responsibilities
  • Assists in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
  • Teaches, supervises, and counsels the family and client in the physical therapy program.
  • Communicates with other team members and, when appropriate, instructs them in certain phases of physical therapy that they may use while working with the client.
  • Writes clinical/progress notes on the date of the visit and incorporates same in the client's record according to company policy.
Requirements
  • Graduate of an accredited Physical Therapy program or equivalent education.
  • Licensed to practice as a PT in the State of Florida.
  • Home healthcare experience is preferred but not essential.
  • Electronic medical record experience is preferred but not essential.
