We are seeking a dedicated and compassionate Physical Therapist to join our team of skilled home health care providers. As a Physical Therapist at Pinnacle Home Care Inc., you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Job Summary

The Physical Therapist is a professional member of the healthcare team who provides Physical Therapy services to patients in compliance with all applicable practice acts. The ideal candidate will have a strong commitment to providing quality care to our patients in their homes.

Responsibilities
  • Assist in the development and implementation of the interdisciplinary client care plan as it pertains to Physical Therapy.
  • Teach, supervise, and counsel the family and client in the physical therapy program.
  • Communicate with other team members and instruct them in certain phases of physical therapy that they may use while working with the client.
  • Write clinical/progress notes on the date of the visit and incorporate same in the client's record according to company policy.
Requirements

To be successful in this role, you must have:

  • Graduate of an accredited Physical Therapy program or equivalent education.
  • Licensed to practice as a PT in the State of Florida.
  • Home healthcare experience is preferred but not essential.
  • Electronic medical record experience is preferred but not essential.
