We are seeking a dedicated and compassionate Physical Therapist to join our team at Pinnacle Home Care Inc. in Jacksonville, FL.

About the Role

The Physical Therapist will provide high-quality care to our patients in their homes, prioritizing their well-being and satisfaction. As a member of our healthcare team, you will work closely with other professionals to deliver exceptional patient care.

Responsibilities
  • Develop and implement individualized physical therapy plans for patients
  • Conduct thorough assessments and evaluations to identify patient needs
  • Provide skilled physical therapy services to patients in their homes
  • Communicate effectively with patients, families, and healthcare team members
  • Document patient progress and maintain accurate records
Requirements

To be successful in this role, you will need:

  • A graduate degree in Physical Therapy from an accredited program
  • Licensure to practice as a Physical Therapist in the State of Florida
  • Experience in home healthcare or a related field (preferred)
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
