The Nurse Practitioner (NP) at Bristol Hospice is an adept professional equipped with advanced education and clinical expertise. The NP is integral in offering comprehensive hospice services while complying with state regulations and collaborating with a supervising physician.

Key Responsibilities:
  • Assess patients' health histories and promote overall wellness
  • Diagnose medical conditions and manage health concerns

  • Collaborate with patients, families, and healthcare providers to ensure effective hospice care

Essential Job Functions:

- Deliver medical care and manage terminal conditions in accordance with the state Nurse Practice Act
- Conduct thorough evaluations, order diagnostic tests, and document findings in patient records
- Develop personalized care plans, prescribe medications, and coordinate hospice services
- Assess hospice eligibility and consult with the interdisciplinary team
- Collaborate with healthcare professionals to ensure patients receive necessary care
- Educate patients and families on preventive care and end-of-life considerations
- Ensure compliance with healthcare regulations and organizational policies

Work Environment:

- Adhere to company policies, hospice regulations, and anti-discrimination standards
- Foster positive working relationships and effective communication skills
- Maintain a valid driver's license and reliable transportation
- Submit to background checks and uphold CPR certification

Required Education & Experience:

- Graduate from an accredited School of Nursing and Nurse Practitioner Program
- Current nursing licensure in the respective state
- Master's degree with a minimum of one year of NP experience and hospice/palliative care experience
- Certification from an approved body for advanced practice nursing
- Specialized certification in areas such as Hospice and Palliative Nursing is preferred.
