The Advanced Practice Nurse (APN) is a licensed nursing professional with advanced education and clinical expertise who delivers comprehensive hospice services in alignment with state regulations and under the guidance of a collaborating physician.
The APN is responsible for hospice care through thorough evaluations and assessments of health histories, promoting wellness, diagnosing medical issues, managing health challenges by formulating and executing care plans, prescribing necessary medications or treatments, and collaborating with patients, families, and other healthcare professionals.
This role, under physician supervision, involves visiting patients, conducting physical assessments, performing face-to-face evaluations, and determining hospice eligibility in conjunction with the hospice interdisciplinary team.
The APN utilizes community resources to design and implement services tailored to meet the needs of individuals and families within their homes and local environments.

Key Responsibilities:
- Deliver medical care, manage terminal illnesses in accordance with state regulations under physician oversight.
- Conduct thorough history and physical examinations by assessing the patient's medical condition and health background, ordering and interpreting laboratory and diagnostic tests, diagnosing health conditions, and documenting findings in the patient's medical record.
- Manage health conditions and terminal illnesses by creating individualized care plans, prescribing medications and treatments, coordinating hospice services, and obtaining necessary consultations or referrals.
- Complete face-to-face visit requirements to assess hospice eligibility based on established conditions of participation.
- Provide clinical insights to the hospice medical director for eligibility assessments for recertification.
- Review clinical data and oversee patient care in collaboration with the hospice interdisciplinary team and other healthcare providers.
- Engage with the collaborating physician when patient care plans deviate from established protocols.
- Communicate with attending physicians and the hospice medical director regarding patient eligibility and ongoing care needs.
- Educate and counsel patients, families, and caregivers on preventive care and medical issues.
- Work collaboratively with families and the interdisciplinary team to identify care goals and meet the needs of patients and their families.
- Serve as a nursing resource for consultation and education within the interdisciplinary team and the broader healthcare community.
- Maintain compliance with Medicare/Medicaid regulations and state/federal laws governing hospice services.
- Ensure adherence to HIPAA regulations and organizational policies.
- Participate in quality monitoring of medical services.
Requirements:
- Must possess a degree from an accredited School of Nursing and an accredited Nurse Practitioner Program.
- Current nursing licensure in the state is required.
- A master's degree with at least one year of Nurse Practitioner experience and one year of hospice or palliative care experience is essential.
- Accreditation from an approved certifying body for advanced practice nursing is necessary.
