Job Summary

The primary purpose of this role is to provide high-quality patient care, promoting comfort, dignity, and well-being. As a Certified Healthcare Associate, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident.

Responsibilities
  • Provide direct patient care, including assistance with daily living activities, medication administration, and mobility support.
  • Conduct thorough assessments of residents' physical and emotional needs, documenting findings and implementing interventions as needed.
  • Develop and maintain strong relationships with residents, families, and healthcare professionals to ensure seamless communication and coordination of care.
  • Collaborate with the care team to identify and address potential safety risks, ensuring a safe and supportive environment for all residents.
Requirements
  • Certified Nursing Assistant (CNA) certification in good standing.
  • High school diploma or equivalent required; prior experience in long-term care preferred but not necessary.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Able to lift 25 pounds and push/pull up to 200 pounds, with regular use of stairs and mobility equipment.

  • Base salary: $45,000 - $60,000 per year, depending on experience.
